SUMMARY: Provide for the total health care needs of Adult Day Program Clients

Job Title: Program Nurse (LPN)

Department: Adult Day

Supervisor: Program Director

Hours: Per Diem



Job Standards

1.Interact in a professional manner at all times, exhibiting friendly behavior when working with clients, family members, collaborators, the general public, and co-workers

2.Report to work each day at scheduled time

3.Complete all mandatory trainings each year

4.Adhere to lunch and/or break schedules


Essential Job Functions

1.Conduct new client intake and evaluations to determine the appropriateness of referrals, including nutritional, medical, physical, and cognitive functional needs

2.Participate in interdisciplinary care plan meetings upon initial intake, bi-annually and on an as needed basis

3.Maintain client confidentiality and respect client dignity

4.Case manage health issues or illnesses, including but not limited to verbal and written contact with physicians and other health care agencies and dispensing medications while utilizing the strict medication distribution protocol

5.Coordinate all on-site medical or health care emergencies

6.Comply with procedures involving suspected abuse, neglect, and exploitation

7.Supervise clients at lunch, monitoring diets and eating patterns

8. Responsible for chart documentation, including but not limited to intake information, progress notes, behavioral notes, physicals, care plans and PPD related information

9. Supervise Program in the absence of the Program Director

10.Assist in the selection and training of new nursing staff

11.Maintain regular attendance; perform other related duties as assigned



Minimum Qualifications:

Education: Graduate of an accredited nursing school

Experience: 2-3 years of experience working with the elderly and those with Dementia/Alzheimer's Disease. Preferred work experience in a community-based setting, and knowledge of services and entitlements for the elderly are a plus

Licensure: State of Connecticut LPN license; CPR certification
