Job Title RN/LPN - Clinical Liaison Home Health & Hospice

Department: Home Care

Supervisor: Director, Home Care & Hospice

Job Objective: Under the direction of the Director, Home Care & Hospice, the Registered Nurse(RN)/Licensed Practical Nurse (LPN) provides services in accordance with agency policy. Provides nursing care to assigned clients under the direction of a RN case manager where applicable. Provides nursing care as defined within the Nurse Practice Act and Friedman Home Care Agency polies. Supports patient care with intake and referrals, meeting with clients and families, and addressing client and provider concerns.

Essential Job Functions

1.Nursing Care: Provides direct nursing care, personal care, treatment and administration of medications to assigned patients under the direction of a Registered Nurse. Complies with infection control standard precautions and safety instructions. Documents clinical notes in an efficient and accurate manner. Provides direct nursing care, personal care, treatment, and administration of medications to assigned patients under the direction of a Registered Nurse.

Provides accurate information to health care professionals involved in the total care of the client. Keeps current with CEUs per states regulation.

2.Intake and Referral: Takes new client and patient referrals from hospitals, senior living communities, nursing homes, physicians, families and others. Establishes and maintains effective interpersonal relationships with clients and families, nursing staff and other disciplines to effectively start services and/or address changes in care needs.

3.Customer Service: Work with team to provide superior customer service including communicating with clients, families, providers, and team members on new referrals, care needs, and/or issues. Support Coordinator, Home Care with referral support including data entry, follow-up calls and other duties as needed.

4.Other duties as assigned by Director, Home Care & Hospice.


Minimum Qualifications

EducationGraduate from an accredited school of nursing. Current valid RN/LPN license with the State Board of Nursing. Must meet all continuing education hours and education requirements. Ability to read, write and communicate in English.

ExperienceMinimum of one year's work experience as an RN/LPN; home health/hospice experience preferred

LicensureRN/LPN, State of Connecticut, CPR certification
