Summary: Responsible for client transportation and coordination of transportation services

Hours: Full-time 30 hrs./flex

Mon - Fri


1.Drive set routes and perform individual door-to-door pick ups of adult day clients according to daily schedule

a.Assist clients with entering and exiting vehicle during pick-up and drop-off at destinations

b.Properly operate vehicle wheelchair lift to ensure safety

c.Maintain daily driving logs, mileage record and other documentation as required

2.Transport clients to and from recreational out trips, or other appointments as scheduled

3.Schedule and coordinate driving routes

4.Coordinate maintenance of vehicles

5.Act as consultant to Program Director to develop additional transportation business

a.Identify potential business partners

b.Act as liaison, alongside Program Director, to potential business partners

c.Assess appropriate utilization of vehicles to maximize usage

6.Act as lead transportation contact between drivers, family members, and Program Director, regarding day-to-day transportation needs, and/or inclement weather specific instructions as determined by and with Program Director.

7.All other related duties as assigned.


Minimum Qualifications:

Education: High School Diploma/GED

Experience: Five year experience working with the elderly and their physical demands and behaviors associated with dementia/Alzheimer's Disease; five years of experience in driving field with scheduling, coordination, and business growth capabilities.

Licensure: Valid Driver's License with no points for five years. Must possess a current Connecticut CDL license with and F endorsement; must posses a current Medical Examiners Certificate from the State of Connecticut.
