Job Title: Coordinator, Home Care Services
Department: Home Care
Supervisor: Administrator, Home Care

Job Objective: Under the direction of the Home Care Agency Administrator, supervise Home Health Aides and support the Home Care Department.
 

Minimum Qualifications

Education: RN preferred
Experience: 3-5 years previous related field
Licensure: Valid Motor Vehicle Driver's Licence
 

 

Job Standards
 

1.     Ability to participate as key member of home care business development, and identify new business opportunities to provide services and excellent customer service.

2.     Good computer, organizational, oral and written communication skills. 

3.     Ability to meet deadlines and handle multiple tasks.

4.     Oversee quality of all aide care.

5.     Ability to manage staff, clients and families. 

6.     Requires working some flexible hours.

 

Essential Job Functions
 

1.     Business development and Client Services including: Attend relevant community events to promote services.  Promote, inform and follow up with potential clients of service opportunities and service agreements, address client issues/concerns in a timely and professional manner, provide ongoing communications to clients and relevant family members.

2.     Answer questions and provide information to prospective clients, resolve client, provider and care giver issues, calling supervisor as back up when necessary.

3.     Oversee Office Operations including: manage, recruit, coordinate orientations and train/mentor staff, supervise assigned aides and ensure competence, ensures timely documentation of all visits, ensures progress reports are made to physicians as necessary.  Creates and maintains a professional working environment for staff, fostering collaborative working relationships for external/internal staff, clients and community. Ensures required in-service education is completed for staff.  Perform on call as needed and collaborates with Coordinator, Home Care and HC Administrator.
